PROJECT DIAL
Project
DIAL, Diabetes Information & Action for
Latinos, was established by a three-year grant
from the Connecticut Health Foundation to
the Connecticut Association for United Spanish
Action, Inc. (C.A.U.S.A.). Since diabetes
is one of the top health problems in the Latino
community, Project DIAL’s mission is
to:
Help
members of the Connecticut Latino community
to identify if they have or are at risk
of having diabetes.
Assist Connecticut Latinos
who have diabetes or have family members
with the disease to better manage their
diabetes on a daily basis.
Assist Connecticut Latinos
who have diabetes to obtain medical care
and supervision that supports their efforts
to effectively manage their disease.
Project DIAL is truly a
grassroots, collaborative effort marshalling
the strengths and capabilities of a variety
of Connecticut health, social service and
government agencies, among them:
American Diabetes Association,
Connecticut Chapter
Amigos de Salud
Hartford Hospital
Connecticut Primary
Care Association
Connecticut Department
of Public Health, Diabetes Control Program
Connecticut Community
Health Services
CAUSA member agencies
Hispanic Health Council
Yale-Griffen Diabetes
Prevention Research Center
Burns Elementary School,
Hartford School System
Project DIAL’s
goal is to provide Connecticut Latinos who
have diabetes the ability to enjoy a longer
and better quality of life.
